Partnership for Early Access for Kids

Solano County integrated its First 5 and Mental Health Services Act planning and funding to support PEAK – Partnership for Early Access for Kids – a high-impact, measurable system of services aimed at improving the health and school success of young children.

PEAK is a countywide, multi-agency collaborative with the overarching goal of earlier identification of infants and children with special needs in order to link them with needed services prior to reaching school age, thus improving their developmental potential.  This innovative collaborative coordinates across disciplines and maximizes resources to ensure at-risk children are identified and treated early, potentially generating millions in “back-end” savings for both county health departments and Special Education Local Planning Areas (SELPAs).
PEAK is supported jointly by First 5 Solano and Solano Mental Health Services Act funds of approximately $1,000,000 annually.  In addition, a First 5 Solano grant of $300,000 serves as the local Early Periodic Screening Diagnosis and Treatment (EPSDT) match to leverage a $2,000,000 in EPSDT treatment services for Medi-Cal-eligible children.  The three-year PEAK operating budget totals approximately $3,138,500.  PEAK is funded through June 2012.  PEAK strategies include:
  • Parent and Caregiver Education and Training –Parent/caregiver workshops, coordination with neighborhood  based Center Based Organizations (CBO's) andFamily Resource Center trainings, targeted outreach and training for parents and child care providers of young children in subsidized child care. 
  • Provider Education and Training – Training in developmental milestones, social-emotional and developmental screening via research-based tools such as the ASQ 3, ASQ-SE and M-CHAT for pediatric care providers, child care providers, Child Welfare Services and others serving high-risk, stressed families.
  • Screening and Assessment – Community outreach countywide to screen at least 700 infants and toddlers annually, with follow up assessment for approximately 21% per year with identified social emotional and/or developmental concerns.
  • Parent Coaching -- Evidence-based parent coaching (Nurturing Parent Program, Parent Child Interactive Therapy (PCIT) and Incredible Years) in both one-on-one/home-based and in group/center-based models, targeting stressed families (ex: incarcerated parents and parental substance abuse).
  • Treatment Services – EPSDT services for eligible children as well as short-term treatment for children identified as at-risk who do not qualify for funding under Medi-Cal/EPSDT or other sources.
  • Coordination – All direct client services (Screening and Assessment, Parent Coaching, and Limited Treatment) in the Initiative are referred and triaged through the Coordination function via a special referral line (phone 707-422-BABY).  www.PEAKSOLANO.org
